The Council on Aging is staffed with a team of licensed professional social workers who specialize in working with older adults.  The social work staff assists close to 2,000 clients every year. They conduct in-home visits to assess each person’s individual strengths and functional abilities, help them plan for assistance now and in the future, make connections with service providers and informal resources, monitor the success of the plan, and make necessary adjustments as time goes on. They are ready to work with any Lake County senior and/or families and caregivers of these older adults.  Our social work staff works closely with many Lake County social service, healthcare, and other aging service providers to ensure that assistance is well-coordinated and complete.

As the older adult population in Lake County continues to grow, we have seen an increase in clients who have more intensive care needs. In response to this need, social work staff works to assist clients who may be facing multiple challenges such as physical, cognitive, emotional, and functional limitations. Our social workers work closely with seniors and their caregivers to ensure safety, and to navigate the maze of senior services.
