The Council on Aging has five full-time case managers, all of whom are licensed social workers. These case managers assist close to 2,000 clients every year. They conduct in-home visits to assess each person’s individual strengths and functional abilities, help them plan for assistance now and in the future, make connections with service providers and informal resources, monitor the success of the plan, and make necessary adjustments as time goes on. They are ready to work with any Lake County senior and/or families and caregivers of these individuals. They work closely with major Lake County senior service providers to ensure that assistance is well-coordinated and complete.
