The Lake County Council on Aging is seeking a dynamic and qualified individual to join Social Work Services. This position is responsible for providing a full spectrum of social work services for older adult clients, their families and caregivers, which will include conducting initial assessments, developing, implementing, evaluating, and revising comprehensive care plans; referring clients to appropriate service providers; conducting follow-up, as needed; and coordinating for quality service delivery. All activities must be professionally documented.
Reports to: Social Work Manager
Hours: Full-time, 8:30am – 4:30pm M-F, Non-Exempt
Education: Bachelors or Master’s Degree in Social Work or closely related field. LSW or LISW in Ohio is required.
Experience: Minimum of two years of social work experience is preferred, but not required.
