Geriatric Assessment Program

The Lake County Council on Aging is proud to partner with Lake Health to offer The Geriatric Assessment Program (GAP). This innovative collaboration provides outpatient, multidisciplinary clinical assessments for seniors who may be struggling with functional issues. Common concerns include: memory loss, confusion, declining ability to manage activities of daily living such as medication management, bill paying, grooming and hygiene, increased falls, mood and behavior changes, and others.

Prior to an initial appointment, our program coordinator, who is a social worker, gathers information about an individual’s social and medical history, current challenges, support system, and functional abilities. Our patients are initially seen and evaluated by a geriatrician and clinical health psychologist at our clinic at the Lake Health SOM Center Campus in Willoughby. An occupational therapist then visits the patient at home to complete an in-home safety assessment. If the patient is still driving, he or she may participate in a clinical driving evaluation. Lastly, a team meeting with the patient and his or her support system offers the opportunity to discuss clinical outcomes and recommendations. If the patient lives in Lake County and ongoing support is needed, he or she can be linked seamlessly with the Council on Aging for services.

Our goal is to provide a comprehensive look at medical, cognitive, functional, and social functioning to give patients and their families the information and resources they need to stay independent in the community and plan for the future.
